Medial compartment

·      adductor compartment

·      separated from extensor compartment by medial intermuscular septum

·      not separated from posterior compartment

adductor muscles
Medial intermuscular septum

·      lies deep to femoral vessels

·      continuation of fascia on pectineus

·      ie. fascia lata on floor of femoral triangle

·      continues down on floor of adductor canal

·      medial edge attached to fascia lata

·      lateral edge attached to linea aspera

Gracilis

Origin

·      inferior ramus of pubis

Path

·      becomes cylindrical tendon

Insertion

·      pes anserinus

Nerve supply

·      obturator n (anterior branch).

Adductor longus

Origin

·      body of pubis

·      angle between pubic crest and symphsis

Path

·      starts as round tendon

·      quickly becomes muscular

·      separates femoral a and profunda femoris

Insertion

·      lower 2/3 of linea aspera

Nerve supply

·      obturator n (anterior branch)

Adductor brevis

Origin

·      body and inferior ramus of pubis

Path

·      separates anterior and posterior divisions of obturator n

Insertion

·      linea aspera

·      lateral to pectineus

Nerve supply

·      obturator n (anterior branch)

Adductor magnus

·      fusion of hamstring and adductor

Origin

·      hamstring

·      lateral part of inferior rough part of ischial tuberosity

·      (medial part receives sacrotuberous ligament)

·      adductor

·      ischiopubic ramus

Path

·      femoral vessels exit adductor canal between adductor and hamstring parts

·      most posterior fibres (hamstring) pass vertically

·      more anterior fibres inserted progressively higher

·      most anterior fibres (adductor) pass horizontally

Insertion

·      hamstring

·      adductor tubercle

·      adductor

·      linea aspera and gluteal crest of femur

·      upper fibres edge-to-edge with quadratus femoris

Nerve supply

·      obturator n (posterior branch)

vessels
Profunda femoris

Perforating branches

·      4 perforators

·      upper 2 arise in femoral triangle

·      pass backwards around adductor brevis

·      upper 1 above AB

·      middle 2 through AB

·      lower 1 below AB (is terminal branch)

·      pass through

·      adductor magnus

·      lateral IM septum

·      end in vastus lateralis

obturator structures
Obturator externus

Origin

·      obturator membrane

·      margin of obturator foramen

Path

·      passes laterally and posteriorly under neck of femur

·      capsule of hip joint extends as far as where tendon in contact with neck

·      halfway down neck

Insertion

·      medial surface of greater trochanter

·      into trochanteric fossa

Obturator artery

·      obturator membrane and muscle do not completely fill obturator foramen

·      do not occlude obturator sulcus superiorly

·      obturator artery passes out notch above membrane

·      with nerve

·      divides into medial and lateral branches

·      encircle origins of obturator externus

·      anastomose with MCFA

·      twig from lateral branch runs in ligamentum teres

Obturator nerve

·      runs in notch with artery

·      divides into anterior and posterior branches

·      branches straddle adductor brevis

Anterior branch

·      passes out notch with artery

·      above obturator externus

·      gives branch to hip joint

·      descends behind AL

·      passes over AB

·      gives off cutaneous branches

·      ends in subsartorial plexus

·      supplies muscles

·      AL

·      AB

·      gracilis

·      supplies skin

·      medial thigh

·      above medial knee

Posterior branch

·      passes through obturator externus

·      passes deep to AB on AM

·      supplies

·      obturator externus

·      AM

·      gives branch to knee joint

·      runs with femoral a

·      passes with middle geniculate a
