Introduction
These notes were written by me while I was studying for my fellowship examinations in 1995-6. They are derived from various sources, including texts, journals, lectures, personal communication and original thought. As such, they are not referenced. They are provided on this website as a quick reference, especially for those preparing for their fellowship examinations. I acknowledge the input of Dr Mark Ross, who I studied with, into the sections on Examination. If anyone wishes to contribute to this compilation with a correction, modification, update or new topic, I would most welcome this. Contributions should follow the same general format, and are best e-mailed to me.
