Priority Appointment System

We need to better understand your problem before we organize an appointment time, as we try to give priority to urgent and serious problems.

We need you to download and complete one of the following forms:

Click on the appropriate link to open the forms which best describe your problem, then print them out and fill them in (they are in PDF format and you will need Adobe Reader to view them).

Return the following to us:

  • The completed forms
  • Your GP referral letter
  • Any X-ray and scan reports (please do not send the X-rays and scans themselves, just the report)

To cancel an appointment

Telephone the office during business hours. Please cancel at least 1 day ahead so that your appointment time can be allocated to another patient who is seeking treatment.
